LIBRARY CARDS
Every Cannon County resident age 5 or older is eligible to borrow materials from Cannon County libraries. Library cards are free for eligible patrons. Identification with your name and current address and phone number is required. All patrons must fill out an official application card.
 
  1. Children ages 5 to 16 must have a parent or legal guardian in the presence of library staff with the parent providing proof of address and signature.
  2. Corporate library cards may be issued to group homes, day cares, and etc. The director of the organization must come in the library and fill out  the application, and agree to be responsible for all fines and missing items.
  3. Replacement cards are $2.00 per replacement.
  4. A valid library card is required in order to check out library materials or use public computers. Non Residents may request a guest card for one-time use.

What to bring before picking up your card? Patrons must have a photo ID with current address, if not please provide a piece of mail with current address.

While submission of this form is not mandatory, the CCLS does wish to maintain demographic information about patrons who apply for library cards in order to evaluate services, programs, and activities that will best meet the needs of our patrons.
